Oral cancer screening takes about two minutes. It's part of every check-up at our Maidan Garhi clinic, and you might not even notice we're doing it. Here's why it matters. India accounts for a third of the world's oral cancer cases, largely because of tobacco and gutka use. The frustrating part is that oral cancer caught early has a very high survival rate. Caught late, the outcomes change dramatically. What we look for: white or red patches on the cheeks, tongue or floor of mouth that don't heal in two weeks. Lumps or thickened tissue. Persistent sore throat or hoarseness. A change in how your teeth fit together. Risk factors that warrant extra vigilance: tobacco in any form (smoking or chewing), regular alcohol consumption, age above 45, and a history of HPV exposure. If you use tobacco and have been thinking about stopping, this is a sign. We won't lecture you. We will examine you carefully and refer you to a specialist immediately if anything looks off. A dentist who shows you the X-ray, points out the issue, and explains your options (including doing nothing) is one who respects your decision-making. If everything feels rushed, that's a flag. Ask about technology, but don't be sold on it. Digital X-rays and intraoral scanners are now standard. Lasers, microscopes and 3D printing are useful for specific cases. The presence of fancy equipment matters less than the dentist's skill in using it. Pricing should be predictable. Ask for a written treatment plan with costs before any work starts. Surprises during procedures are not okay. Cleanliness is non-negotiable. Look around. Are instruments sealed and dated? Is the chair clean between patients? Does the staff use proper PPE? Reviews help, but read the negative ones too. A clinic that handles a complaint well is often better than one with only five-star praise. Most people put off teeth cleanings because they don't really know what happens, and the sound of the ultrasonic scaler is intimidating. So here's the unfiltered version. You sit back. The hygienist examines your gums first, noting any bleeding spots or pockets deeper than they should be. The ultrasonic scaler vibrates at high frequency to break up hardened tartar (calculus) along the gum line. It feels like a strong tickle. Not painful, sometimes ticklish enough that people laugh during it. Cold water sprays to keep things cool. Hand instruments finish the spots the scaler couldn't reach, especially between teeth. Polishing uses a soft rubber cup with a mildly gritty paste. This part feels nice. A final fluoride application strengthens enamel. Total time at our Delhi practice: about 30 minutes. You leave with cleaner teeth and a slightly tingly feeling at the gum line that fades within an hour. If it's a small chip (no pain, smooth edge): you can wait a few days for an appointment, but don't leave it indefinitely. Sharp edges cut your tongue and the exposed dentin can stain or get sensitive. If it's a larger piece (pain when you bite, sensitivity to cold): come in within a day or two. The tooth needs covering before the nerve becomes irritated. Bring the broken piece if you have it, sometimes we can bond it back. If the tooth has broken at the gum line or you can see what looks like pinkish tissue inside: this is urgent. The pulp is exposed and needs protection within 24 hours, ideally faster, to avoid a root canal becoming necessary. Cracks from grinding are a separate issue. Missing a single tooth and weighing a bridge against an implant? Both restore the gap. They work differently. A bridge uses the two teeth on either side of the gap as anchors. We shape those teeth down and fit a three-unit crown over them, with a fake tooth (called a pontic) suspended in the middle. The work is fast, often done in two to three visits. An implant replaces just the missing tooth, with no involvement of the neighbours. The titanium post goes into the bone, heals over a few months, then we fix the crown on top. When a bridge makes sense: the adjacent teeth already have large fillings or crowns. Bone is poor and grafting would be needed for an implant. You need a fast solution, perhaps within a month, and don't want to wait for healing. When an implant makes sense: the neighbouring teeth are healthy and you want to keep them untouched. You're young and the long-term care of an implant suits you better. You have time and bone for proper healing. A clicking or popping sound when you open wide. Difficulty opening fully (less than three fingers stacked vertically between your front teeth). Pain when you yawn. Knocked out a tooth playing cricket. Cracked a molar on a hidden chana. Swelling around a tooth that started this morning and is now spreading to your cheek. What do you do before you can get to a dentist? Knocked-out tooth: hold it by the crown (not the root), rinse gently with milk or saline (not water if you can avoid it), and either put it back in the socket or carry it in a glass of milk. Get to a dentist within an hour for the best chance of saving it. Cracked tooth with pain: don't chew on that side. Rinse with warm salt water. If a fragment is loose, save it. Call us. Facial swelling: this needs same-day attention. A dental infection that spreads to the cheek or under the jaw can become serious quickly. Severe throbbing pain: cold compress on the outside of the cheek, paracetamol if you can take it. Avoid placing aspirin directly on the gum, which burns the tissue. Show as metallic color. Used mostly on back molars where they don't show in your smile. Porcelain-fused-to-metal (PFM). Tooth-coloured outside, metal core inside. Reliable, mid-range cost. Over years, the gum line can recede and show a dark metal margin. All-ceramic (zirconia or E-max). Fully tooth-colored, no metal. Zirconia is exceptionally strong, suitable for back teeth and high bite forces. E-max gives the most natural light reflection, ideal for front teeth. For most patients, our recommendation at the Chhatarpur Maidan Garhi practice is zirconia for back teeth (strength) and E-max for front teeth (aesthetics). Same-day crowns are possible in some cases.
